Feritin Cairan Serebrospinal sebagai Biomarker untuk Membedakan Meningitis Bakterialis dan Meningitis Tuberkulosis pada Anak

Latar belakang. Meningitis adalah salah satu penyakit yang memiliki tingkat mortalitas dan morbiditas tinggi. Setiap jenis meningitis memiliki karakteristik masing-masing, tetapi gejala dan tanda klinis serta temuan laboratorium sering tumpang tindih. Feritin sebagai penanda biologis potensial yang diduga dapat membedakan antara meningitis bakterialis dan tuberkulosis untuk pemberian antibiotik secara cepat dan tepat.
Tujuan. Membedakan kadar feritin cairan serebrospinal (CSS) antara meningitis bakterialis dan meningitis tuberkulosis pada anak.
Metode. Penelitian komparatif potong lintang di RSUP Dr. Hasan Sadikin Bandung pada Maret 2019. Bahan penelitian menggunakan CSS bahan biologis tersimpan anak usia 1 bulan−18 tahun yang masing-masing telah didiagnosis meningitis bakterialis dan tuberkulosis. Kadar feritin CSS diperiksa menggunakan metode two-site sandwich immunoassay. Data dianalisis menggunakan uji Mann-Whitney, tingkat kemaknaan hasil uji ditentukan berdasarkan nilai p<0,05.
Hasil. Di antara 46 bahan penelitian, 24 laki-laki, median usia 7 tahun (1 bulan−17 tahun). Dari data karakteristik tidak didapatkan perbedaan yang bermakna. Terdapat perbedaan bermakna kadar feritin CSS meningitis bakterialis dan tuberkulosis (p<0,001). Median kadar ferritin CSS pada kelompok meningitis bakterialis 4,18 ng/mL(1,30–24,26), lebih rendah dibandingkan pada kelompok tuberkulosis sebesar 15,47 ng/mL dengan rentang (1,60–1.069,96).
Kesimpulan. Terdapat perbedaan bermakna kadar feritin CSS antara kelompok meningitis bakterialis dan meningitis tuberkulosis pada anak.
